In the United Kingdom women have access to termination of pregnancy for maternal reasons until 24 weeks’ completed gestation, but it is accepted practice for children born at or beyond 25 weeks’ gestation to be treated according to the child’s perceived best interests even if this is not in accordance with parental wishes. BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. John Locke's earliest significant publications appeared between 1686 and 1688 in the Bibliothèque universelle et historique. They were a translation of his New Method of a Commonplace Book, an abridgment of his (as yet unpublished) Essay Concerning Human Understanding, and two reviews, of a medical work by Robert Boyle, and Isaac Newton's Principia. It is likely that he contributed some other book reviews, but these cannot now be identified. An examination of surviving copies of the Bibliothèque universelle et historique (...) shows that it had a very complicated printing history, and both the volumes (tom. 2 and tom. 8) that certainly contain items by Locke were reprinted on several occasions, in some cases in editions that probably have false dates and imprints. Though this article concentrates mainly on the two volumes known to contain material by Locke, a preliminary survey of entire printing history of the journal has also been made, with the results presented in tabular form in Appendix I. Since all of the distinguishing features of the early development of modern physical science seem to be embodied in the works of Newton, e.g., the abhorrence of occult qualities and the great surge of experimental knowledge, the mechanical view of matter explained by mathematical theory, the constant attempt to reconcile the God of revelation with the world machinery, Robert Boyle has too often been overlooked. In addition to giving a short sketch of Boyle's life, Mrs. Hall has admirably (...) selected texts from the eighteenth century Birch edition which at once manifest the genius of Boyle and his influence on Newton. Each selection is adequately explained and commented upon by Mrs. Hall in Part One. The chapter headings in the author's introductory essay parallel the headings she has given to the selections in Part Two: "The New Learning and its Method," "The Mechanical Philosophy," "Chemistry," and "Pneumatics." The sections on mechanical philosophy contain the core of Boyle's natural philosophy. The characteristic blend of natural philosophy with natural religion which was common to many members of the Royal Society led Boyle to disdain the Baconian attempt to banish teleology from investigations. Boyle's view of a world machinery is never considered as a scientific hypothesis as that term is understood today, but rather it appears to be at times a theological doctrine. The mechanical theory itself—the corpuscular theory of matter—which Boyle promised to set down in systematic form was actually never written as a complete system. Mrs. Hall has done much to give systematic coherence to Boyle's corpuscular theory in her excellent commentary and selection of texts. The book contains an adequate index, bibliographical notes, and a partial list of Boyle's works. One with an historical interest in the Newtonian age will discover another giant whose shoulders made it all possible.—J. J. R. (shrink)

This paper summarizes ideas about the nature of gases proposed during the period following the discovery of Boyle's law. Particular stress is laid on the hypotheses of the Bernoullis, and later, on the equally speculative work of Herapath. Reasons for the success of Herapath's theory, and the neglect of Daniel Bernoulli's are discussed, but it has not been thought necessary to take the story beyond the initial acceptance of Herapath's theory by J. P. Joule, because the paper is concerned (...) only with the antecedents of the modern kinetic theory. (shrink)

Political liberals ask citizens not to appeal to certain considerations, including religious and philosophical convictions, in political deliberation. We argue that political liberals must include a demanding requirement of intellectual modesty in their ideal of citizenship in order to motivate this deliberative restraint. The requirement calls on each citizen to believe that the best reasoners disagree about the considerations that she is barred from appealing to. Along the way, we clarify how requirements of intellectual modesty relate to moral reasons for (...) deliberative restraint. And we argue against attempts to weaken our requirement of intellectual modesty by emphasizing those moral reasons. (shrink)

Determining what has gone wrong in a malfunctioning body and proposing an effective treatment requires expertise. Since antiquity, philosophers and doctors have wondered what sort of knowledge this expertise involves, and whether and how it can warrant its conclusions. Few people were as qualified to deal with these questions as Galen of Pergamum. A practising doctor with a keen interest in logic and natural science, he devoted much of his enormous literary output to the task of putting medicine on firm (...) methodological grounds. At the same time he reflected on philosophical issues entailed by this project, such as the nature of experience, its relation to reason, the criteria of truth, and the methods of justification. This book is a new edition of a short but fascinating treatise by Galen on causal theory. This text survives only in a Latin translation of the fourteenth century, and it is this which appears here. The volume also contains the first translation of the treatise into any modern language, and the first philosophical commentary thereon. The commentary ranges widely in Galen's voluminous œuvre, and compares his views with those of other ancient theorists. The relationship between the morphology of the physical organs and their activities has long been a vexed issue in the philosophy of biology: the question of whether structure determines function is of course of contemporary importance in evolutionary theory. That there was a relationship between structure and function (...) was not in general a matter of doubt to the Greek theoretical biologists: but both the extent, and crucially the direction, of the dependence was. And the question need not be restricted to a consideration of the function of a particular part in isolation: when Aristotle takes issue with Anaxagoras for asserting that humans are the most intelligent species because they have hands, claiming rather that they have hands because they are intelligent, he is making a claim not just about the direction of explanation involved, but he is also implicitly at least indicating the scope of such explanations. For Aristotle, intelligence is one of the human functions: and that function necessitates the existence and development of certain organic structures. (shrink)

Ethics position theory (EPT) maintains that individuals’ personal moral philosophies influence their judgments, actions, and emotions in ethically intense situations. The theory, when describing these moral viewpoints, stresses two dimensions: idealism (concern for benign outcomes) and relativism (skepticism with regards to inviolate moral principles). Variations in idealism and relativism across countries were examined via a meta-analysis of studies that assessed these two aspects of moral thought using the ethics position questionnaire (EPQ; Forsyth, Journal of Personality and Social Psychology39, 175–184, 1980). This review identified 139 samples drawn from 29 different countries, for a total sample of 30,230 respondents, and concluded that (a) levels of idealism and relativism vary across regions of the world in predictable ways; (b) an exceptionist ethic is more common in Western countries, subjectivism and situationism in Eastern countries, and absolutism and situationism in Middle Eastern countries; and (c) a nation’s ethics position predicted that country’s location on previously documented cultural dimensions, such as individualism and avoidance of uncertainty (Hofstede, Culture’s Consequences: International Differences in Work-Related Values, 1980). Limitations in these methods and concerns about the validity of these cross-cultural conclusions are noted, as are suggestions for further research using the EPQ. (shrink)

Empathy is a lay term that is becoming increasingly viewed as a unitary function within the field of cognitive neuroscience. In this paper, a selective review of the empathy literature is provided. It is argued from this literature that empathy is not a unitary system but rather a loose collection of partially dissociable neurocognitive systems. In particular, three main divisions can be made: cognitive empathy , motor empathy, and emotional empathy. The two main psychiatric disorders associated with empathic dysfunction are (...) considered: autism and psychopathy. It is argued that individuals with autism show difficulties with cognitive and motor empathy but less clear difficulties with respect to emotional empathy. In contrast, individuals with psychopathy show clear difficulties with a specific form of emotional empathy but no indications of impairment with cognitive and motor empathy. (shrink)

This study investigated the performance of boys with psychopathic tendencies and comparison boys, aged 9 to 17 years, on two tasks believed to be sensitive to amygdala and orbitofrontal cortex func- tioning. Fifty-one boys were divided into two groups according to the Psychopathy Screening Device (PSD, P. J. Frick & R. D. Hare, in press) and presented with two tasks. The tasks were the gambling task (A. Bechara, A. R. Damasio, H. Damasio, & S. W. Anderson, 1994) and the Intradimensional/ (...) Extradimensional (ID/ED) shift task (R. Dias, T. W. Robbins, & A. C. Roberts, 1996). The boys with psychopathic tendencies showed impaired performance on the gambling task. However, there were no group differences on the ID/ED task either for response reversal or extradimensional set shifting. The implications of these results for models of psychopathy are discussed. This study investigates the ability of individuals with psychopathy to perform passive avoidance learning and whether this ability is modulated by level of reinforcement/punishment. Nineteen psychopathic and 21 comparison individuals, as defined by the Hare Psychopathy Checklist Revised (Hare, 1991), were given a passive avoidance task with a graded reinforcement schedule. Response to each rewarding number gained a point reward specific to that number (i.e., 1, 700, 1400 or 2000 points). Response to each punishing number lost a point punishment specific (...) to that number (i.e., the loss of 1, 700, 1400 or 2000 points). In line with predictions, individuals with psychopathy made more passive avoidance errors than the comparison individuals. In addition, while the performance of both groups was modulated by level of reward, only the performance of the comparison population was modulated by level of punishment. The results are interpreted with reference to a computational account of the emotional learning impairment in individuals with psychopathy. (shrink)
